WAIVER

The Center has combined the adult and junior waivers into one.

Any player wanting to use the Center for sports must complete the waiver beforehand, whether that player is a member or a guest.

Any junior wanting to use the Center for sports must have a parent or guardian complete the waiver beforehand, whether the junior is a member or a guest.

​

These documents are kept on file and do not have to be completed each year of membership or each time a player comes as a guest of a member. The exception to this is if there is a significant change to the waiver content.
